Health Office
Jen Garofalo - Nurse
Welcome to the Nurse's Health Office Website. Information is provided here for your reference, so we may work in a cooperative effort regarding your child’s health. Please contact the school nurse with information, questions or concerns. If the nurse is unavailable, kindly leave a message and she will return your call as soon as possible. Please do not leave an important message that needs to be addressed immediately. If there is an emergent situation, please contact the front office.
Chronic health conditions, such as asthma or severe allergies, needs to be communicated to the school nurse. Open communication helps to ensure that health issues are addressed appropriately and timely. Health forms are provided below for your convenience. They must be completed and signed by child’s physician in order for the school nurse to administer any prescription or over- the -counter medication.
If there are changes in your child’s health, including new allergies, medication, hospitalization, fractures, corrective lenses or any other pertinent information, please inform the school nurse. Many times important accommodations need planning and implementing to ensure the safety and comfort of your child.
Regular school attendance has a huge impact on a student’s academic success. Your child is less likely to succeed if he/she is chronically absent. While we understand and encourage that your child remain absent when ill, it is imperative that they attend school when well. Please reach out to the school nurse or guidance counselor should you need assistance or support.
By working together, I know we can make this a safe, healthy and fun school year for your child.
